The family gives this page’s address to the school; each teacher sends their own form. Answers reach the Admissions Office directly and are never shown to the family.
Years 3 to 6
The main classroom teacher completes one form.
Year 7 and above — English
The English teacher completes one form.
Year 7 and above — Mathematics
The Mathematics teacher completes a second form.
